Acupuncture Coverage

Overview:

Function can bill acupuncture services for in-network benefits within Target employee health plans. Most plans offer 24 visits per year, with no specified cadence or frequency of visits.*

You are ultimately responsible for payment until the billed session is paid in full - a health spending debit card (or card of choice) is required to have on file to charge your coinsurance/copay and any deductible or remaining balance after determination. Balances due will be billed to this card subsequent to your claim decision and Explanation of Benefits (EOB).

*As the subscriber, it is your ultimate responsibility to understand and ensure your coverage, deductible, co-pay and/or coinsurance responsibility.

Process:

  1. Schedule your Acupuncture visit and enter “Target Insurance” in the notes so we can send you a form to gather insurance details.

  2. Use your health debit, flex debit, or card of choice to book and hold your session. This card will not be charged at the time of booking.

  3. Provide insurance information by filling out the form sent via “Jane” after your booking.

  4. Arrive for service. Copay/coinsurance will be billed at this time.

  5. Following your visit, the claim will be submitted electronically to your health plan.

  6. Function will receive a determination (EOB) with payment advice for your claim.

  7. If your contract deductible is not yet satisfied, or the claim is denied as a result of plan details, your card on file will be charged the balance remaining.

  8. Once all billing and benefits processing are complete, a receipt will be provided. Further documentation can be requested at any time.

Things to consider:

Deductibles:

Acupuncture services fall within your normal individual and/or policy deductible. By design, many high-deductible health insurance plans integrate health spending debit cards for proactive, preventative, and general wellness concerns. As you build toward satisfying your deductible, your health spending card is able to pay for these services.

Blue Cross Blue Shield of Minnesota Plans - 20% Coinsurance

Bind/Surest Plans - $40 Copay
